Residential Support Services (RSS) & Supervised Living (SL)

Residential Support services help individuals live safely and comfortably in community-based homes located in established, attractive residential neighborhoods. Our 3-to-4 person group homes provide around-the-clock direct support staff who assist with personal care, meal preparation, medication administration, health maintenance, community outings, and personal budgeting.

Supervised Living (SL): Provides direct support staff present on-site during overnight hours and peak times, ideal for participants seeking greater daily autonomy while retaining safety oversight.

Supported Home Living (SHL) – Own / Family Home

Supported Home Living provides flexible support to individuals who choose to live in their own private home or with their family. Direct support professionals assist with daily living tasks, personal hygiene, grocery shopping, cooking, transportation to appointments, and participation in recreational community events.

This program allows families to remain the central foundation of their loved one’s living arrangement while receiving certified, dependable professional assistance.

Day Habilitation & Pre-Vocational Training

Our Day Habilitation program offers a vibrant, structured learning environment Monday through Friday. Participants engage in customized skill-building activities, including:

  • Social Skills & Communication: Peer collaboration, self-advocacy, and emotional regulation.
  • Pre-Vocational Development: Work readiness, task completion, and volunteer opportunities at local food banks and parks.
  • Life Skills & Financial Literacy: Money handling, personal health routines, and kitchen safety.
  • Community Exploration: Guided visits to museums, libraries, fitness centers, and cultural attractions.

Door-to-door transportation is coordinated by ABACO Healthcare personnel for scheduled day program hours.

Specialized Clinical Therapies & Behavioral Support

Specialized Therapies under the HCS waiver address functional limitations and improve physical, communication, and sensory independence. Licensed clinicians work directly with each participant and provide guidance to staff and family members:

  • Speech & Language Pathology: Enhancing expressive/receptive communication, dysphagia (swallowing) care, and augmentative communication devices.
  • Occupational Therapy (OT): Improving fine motor skills, sensory processing, adaptive dining, and daily living techniques.
  • Physical Therapy (PT): Maximizing gross motor mobility, gait training, wheelchair transfers, and strength retention.
  • Behavioral Support: Functional behavior assessments, positive behavior intervention plans (PBIP), and direct intervention by certified behavior analysts.
Routine & Emergency Dental Treatment

Maintaining proper oral hygiene is vital for overall health and well-being. The Texas HCS waiver provides up to $2,000 per service year for authorized emergency and routine dental procedures, including diagnostic exams, dental x-rays, cleanings, fillings, extractions, and restorative treatment.

ABACO Healthcare assists in coordinating with specialized dental providers experienced in caring for individuals with developmental disabilities.

Professional Nursing Services (RN & LVN Oversight)

Our Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) deliver clinical oversight, medication delegation, health assessments, and chronic condition monitoring. Our nursing team works in close consultation with each individual’s primary care physician to maintain comprehensive health records and ensure rapid clinical intervention when needed.

Minor Home Modifications

Home modifications provide physical adaptations to the participant’s home that are necessary to ensure health, safety, and independence:

  • Exterior and interior wheelchair ramps
  • Bathroom accessibility (roll-in showers, shower chairs, transfer benches, safety grab bars)
  • Doorway widening to accommodate wheelchairs and adaptive walkers
  • Specialized door hardware, accessible sink controls, and safety lock adaptations

*Note: The Texas HCS program provides a coverage for vital minor home modifications to ensure physical safety and accessibility throughout the residence.

Frequently Asked Questions About HCS

Helpful answers to assist you in navigating the Texas Medicaid waiver system and choosing the right support.

Who is eligible to participate in the Texas HCS Program?

To be eligible for the Texas Home and Community-Based Services (HCS) waiver, an individual must:

  • Have an IQ score of 69 or below, or have an approved related condition with an IQ of 75 or below.
  • Have mild to profound deficits in adaptive behavior.
  • Be eligible for Texas Medicaid benefits.
  • Not be enrolled in another 1915(c) Medicaid waiver program at the same time.
  • Have received an official offer of an HCS waiver slot through their local LIDDA.
Can I transfer from my current HCS provider to ABACO HEALTH CARE?

Yes, absolutely. Under Texas Medicaid rules, you always have the legal right to choose or transfer your HCS service provider at any time without interruption to your services. Simply notify your LIDDA Service Coordinator that you wish to transfer to ABACO HEALTH CARE, or reach out to our intake team directly at (346) 467-5008, and we will coordinate all paperwork and provider transfer meetings seamlessly.

What is the difference between HCS and Texas Home Living (TxHmL)?

The primary difference is residential placement. The HCS program includes 24-hour residential provider-operated group homes (RSS) and Supervised Living options, making it ideal for individuals requiring full-time community residential care. TxHmL is designed exclusively for individuals who live in their own homes or family homes and does not include 24-hour residential placements.

Can family members be hired to provide Supported Home Living or Respite?

In many instances, eligible adult family members (with the exception of spouses or legal guardians of minor children) may be hired as direct support personnel under Supported Home Living or Respite, provided they pass required state background checks and complete mandatory provider training. Contact our office to review your family’s specific options.
